Thank you for your interest in Stillwater Group Homes. At the link below, you will find an application for services and other necessary information pertaining to eligibility. Once your application is completed, please submit via mail or in-person to the SGH office at 904 W. 11th, Stillwater, OK 74074. You can call 405-743-2496 for availability. You can also fax your application to 405-743-2497.
